Death Benefit

Death Benefit Payment (Private Sector)

This is payment made to the beneficiary of a deceased contributor/retiree. Upon the death of a RSA holder, the beneficiary named in the Will or a letter of administration will be eligible to access the balance in the RSA.

REQUIREMENTS

  1. PAL death notification form (Click here).
  2. Two (2) Passport photographs of the deceased and NOK/ Administrator
  3. Confirmation of next-of-kin by the employer is required if the RSA holder passed on while in service. The employer is required to issue a written confirmation stating the NOK on the employment records while attaching stamped passport photograph(s) of NOK(s).
  4. Bank document for confirmation of administrator’s bank account number i.e. bank statement or letter of introduction from the bank (where there are multiple administrators, payment will be made into either an estate account or jointly nominated bank account)
  5. Means of identification of administrator(s) i.e. National ID, Voter’s Card, Driver’s License, International Passport, Letter of introduction from the bank or Notary Public (with stamped passport)
  6. Death certificate or registration of death
  7. Letter of Administration or Will
  8. Signed payment consent and means of identification are required where the registered next-of-kin is not an administrator.
 
 
  • Once all the above documents have been received and certified by the issuing parties, PAL seeks for payment approval from PENCOM.
 
  • Upon receipt of approval, PAL instructs UBA PFC to credit the beneficiary’s bank account.
 

For missing persons, the following are additional requirements to be submitted along with the list stated above (with the exception of death certificate):

  • Police report
 
  • Newspaper publication
 
  • Letter from ex-employer confirming details of the missing RSA holder
 
 

DEATH BENEFIT PAYMENT (PUBLIC SECTOR)

This is payment made to the beneficiary of a deceased contributor/retiree. Upon the death of a RSA holder, the beneficiary named in the Will or a letter of administration will be eligible to access the balance standing in the RSA.

 

REQUIREMENTS

  1. Two (2) Passport photographs of the deceased and NOK/Administrator
  2. PAL death notification form (Click here).
  3. Means of identification of administrator(s) i.e. National ID, Voter’s Card, Driver’s License, International Passport, Letter of introduction from the bank or Notary Public (with stamped passport)
  4. Bank document for confirmation of administrator’s bank account number i.e. bank statement or letter of introduction from the bank (where there are multiple administrators, payment will be made into either an estate account or jointly nominated bank account)
  5. Death certificate or registration of death
  6. Letter of Administration or Will

  8. Next-of-kin will provide the following if the RSA holder was not receiving monthly pension from PAL Pensions before demise
  9. Appointment Letter
  10. Deceased sworn affidavit of age/birth certificate
  11. Stamped Last Pay slip before demise
  12. Stamped Pay slip as at June2004
  13. Stamped Pay slip as at January 2007
  14. Stamped Pay slip as at June 2010
  15. Stamped Pay slip as at December 2013
  16. Stamped Pay slip as at December2017
  17. Stamped Pay slip as at current year
  18. Evidence of transfer of service (where applicable)
  19. Letter of introduction from employer must bear two signatories, state name(s) of NOK(s), have stamped passport photograph(s) of NOK(s), deceased person’s date of birth, date of first appointment, date of death, grade level & step as at demise and all periods stated under item D – H above
  20. PENCOM death notification form (Click here)
 
 
  • Once all the above documents have been received, certified by the issuing parties and RSA consolidated, PAL seeks for payment approval from PENCOM.
 
  • Upon receipt of approval, PAL instructs UBA PFC to credit the beneficiary’s bank account.
