LIST OF DOCUMENTS FOR DATA RECAPTURING (PUBLIC SECTOR)

- Transfer and Acceptance of Service (where applicable)
- Birth Certificate or Declaration of Age
- Staff Identity Card or any one of the following:
  - (a) Permanent Voter’s Card
  - (b) International Passport
  - (c) National Driver’s License
- Staff file Number.
- Letter of Employment
- White background passport photograph
- National Identity Card or Enrolment Slip issued by the National Identity Management Commission, indicating the National Identity Number (NIN). (The PFA Official should view the NIN notified to the individual using the appropriate USSD code e.g. *346# for MTN, AIRTEL and 9 Mobile).
- Promotion Letter and Pay Slip indicating Grade Level and Step as at 30 June 2004 (where applicable).
- Promotion Letter and Pay Slip indicating Grade Level and Step as at January 2007 (where applicable).
- Promotion Letter and Pay Slip indicating Grade Level and Step as at July 2010 (where applicable).
- Promotion Letter and Pay Slip indicating Grade Level and Step as at December 2013 (where applicable).
- Promotion Letter and Pay Slip indicating Grade Level and Step as at December 2016 (where applicable).
- Promotion Letter and Pay Slip indicating current Grade Level and Step (where applicable).
- IPPIS No. (for employees of MDAs under IPPIS). (viii) Certificate of RSA Registration (i.e. Welcome Letter or recent RSA Statement showing name and PIN).

- Staff Identity Card or any one of the following:
  - (a) Permanent Voter’s Card
  - (b) International Passport
  - (c) National Driver’s License
- Letter of Employment or Letter of Appointment
- Birth Certificate or Declaration of Age
- National Identity Card or Enrollment Slip issued by the National Identity Management Commission, indicating the National Identity Number (NIN). (The PFA Official should view the NIN notified to the individual using the appropriate authentication code e.g. *346# for MTN, AIRTEL and 9 Mobile).
- One Passport Photograph on white background.

Lawal Kareem Olatunde is currently the Ag. Head, Benefits Payment Department. He is a self-motivated team player with over 14 years’ experience in Benefits Administration, Operations, Contribution Processing, Account reconciliation, Customer Service, Business Development and Health Management roles.

He studied at Adekunle Ajasin University, Akungba- Akoko where he obtained a B.Sc degree in Chemistry and has a Master’s Degree in Business Administration (MBA) from Obafemi Awolowo University, Ile -Ife. He has attended various trainings and workshops including Competitive Strategy Development Programme at Lagos Business School.

Prior to joining PAL Pensions, he has worked in various capacities with Premium Pension Limited, Mega Lifesciences and Codix Pharma Limited.

Kareem is an Associate member of The Certified Pension Institute of Nigeria.”
